Questionnaire

At the end of each financial year, we require all clients to fill out and sign the questionnaire form.

This is to ensure that you include all the relevant information that we need to complete your financial statements and when signed, gives KTS permission to work on your accounts.

You are able to complete the KTS Annual Financial Statements Questionnaire as an online PDF.

To fill in this online form, you will need the latest version of Adobe Acrobat reader, get the free latest version from this website.

If you wish to submit this form online, or digitally sign it

Please to save it to your desktop first.

To save click on the ‘download’ icon and save to desktop or other file, or click on the 3 dots and choose “open in desktop app’, this will then give you access to the ‘signature’ and the ‘submit’ fields.

When you are finished, you can either choose ‘Submit’ – which will bring up an email which will enable you to send to us at KTS or print, sign, scan, and attach to an email to us at jasmine@kts.co.nz.
